On php5 - you need to alter the opening '<?' tag in lib/Database.php to '<?php'

Also plugins/dbccontrols to be a missing template 'DBC_index.tpl' as well as a file index.php

There does not appear to be a copy of it held anywhere else I can find - most links are dead and the only active ones I could find pointed to phop classes.

If you remove the first item entry in the file plugins/dbcontrols/plugin.xml

ie '<item>
<templateName>dbc_index.tpl</templateName>
<mode>ONCE_PER_DATABASE</mode>
<fileName>dbc/index.php</fileName>
</item>'

The code will run to completion - however I do not know if its a case of gigo.
